Individuals who are using for the SEP for the Operating Aged as well as Operating Disabled should finish the form CMS-L564 together with the relevant Part A or Component B registration type. People entitled to premium-free Part A can not willingly end their Part An insurance coverage. This is not allowed by law. Typically, premium-free Component A ends as a result of: Loss of entitlement to Social Protection or Railway Retired life Board advantages; or Fatality.
